This Privacy Policy explains how Koble Cares, a company organized in Arizona (“Koble Cares,” “we,” “us,” or “our”), collects, uses, and shares information in connection with the website located at joinkoblecares.com (the “Site”).
This Policy applies to the public Site only. It does not apply to the secure Koble Cares platform or dashboard used by partner healthcare practices and health systems, which is governed by a separate written agreement, including a Business Associate Agreement (“BAA”) where applicable.
Koble Cares does not knowingly collect Protected Health Information (“PHI,” as defined by HIPAA) through this Site. Information about how PHI is handled when a healthcare practice uses the Koble Cares platform is addressed in our HIPAA and Patient Data Handling Statement and in the applicable BAA, not in this Policy.
If you are a patient who received a letter or message from your doctor’s office mentioning Koble Cares, please do not send us medical information through this website. Questions about that message, or about sharing your information with a resource partner, should go to your doctor’s office or to us using the contact information in Section 13.

We do not knowingly collect Social Security numbers, government identification numbers, financial account information, or PHI through this Site.
We use Google Analytics and similar tools to understand how visitors use the Site. These tools use cookies and similar technologies to collect information such as pages visited and time spent on the Site.
You can control cookies through your browser settings, and you can opt out of Google Analytics using the Google Analytics Opt-out Browser Add-on. We honor opt-out preference signals such as the Global Privacy Control (GPC) where required by applicable law; when we detect a GPC signal, we treat it as a request to opt out of any sale or sharing of personal information for that browser. Our Site does not respond to older “Do Not Track” signals.
We do not sell your personal information, and we do not share it with third parties for cross-context behavioral or targeted advertising. We have not sold or shared personal information in the preceding 12 months.
We also do not use personal information collected through this Site for profiling that produces legal or similarly significant effects, and we do not collect sensitive personal information (such as precise location, biometrics, or health information) through this Site.
We use administrative, technical, and physical safeguards designed to protect information collected through the Site, including encryption of data in transit and access controls that limit who can view inquiry data. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

As a general rule, we keep contact-form submissions and related correspondence for up to three years after our last contact with you, and analytics data for no more than 14 months, unless a longer period is required by law or needed to resolve a dispute.
You may opt out of marketing communications at any time by using the unsubscribe link in our emails or contacting us using the information in Section 13.
Depending on where you live, state law may give you the right to: confirm whether we process your personal information and access it; correct inaccurate information; delete information you provided or we obtained about you; receive a copy of your information in a portable format; and opt out of the sale of personal information, sharing or processing for targeted advertising, or profiling that produces legal or similarly significant effects. We do not discriminate against you for exercising these rights.

We will verify your identity using the information associated with your inquiry before acting on a request, and we will respond within 45 days (which may be extended once by another 45 days where reasonably necessary, with notice to you). If we deny your request, you may appeal by replying to our response; we will respond to appeals within 45 days, and if the appeal is denied you may contact your state Attorney General. An authorized agent may make a request on your behalf if you provide written permission and we can verify your identity.
California residents: in addition to the rights above, you may request the categories and specific pieces of personal information we have collected, the categories of sources, our business purposes, and the categories of third parties with whom we share it. The categories we collect are described in Section 2; the sources are you and your device; the business purposes are in Section 3; and the third-party categories are in Section 5. We do not sell or share personal information, and we do not knowingly sell or share the personal information of anyone under 16.
Nevada residents: we do not sell covered information as defined in Nev. Rev. Stat. 603A.340. You may still submit a request to opt out of any future sale using the contact information in Section 13.
This Site is intended for a professional audience and is not directed to children. We do not knowingly collect personal information from children under 13, as defined in the Children’s Online Privacy Protection Act (15 U.S.C. §6501; 16 CFR Part 312), and we do not knowingly sell or share the personal information of anyone under 16. If you believe a child has provided us information, please contact us so we can remove it.
The Site may link to third-party websites, including those of resource partners. We are not responsible for the privacy practices of those third parties, and we encourage you to review their privacy policies.
This Site is intended for visitors located in the United States. We do not direct the Site to, or knowingly collect information from, visitors outside the United States. If you visit from outside the United States, you understand that your information will be processed in the United States.
We may update this Privacy Policy from time to time. We will post the revised version on this page with an updated effective date. If we make a material change, we will post a notice on the Site’s home page for at least 30 days and, where we have your email address, notify you by email. Changes take effect on the stated effective date and do not apply retroactively to information collected before that date.
